Key facts

Oseltamivir is a neuraminidase inhibitor active against influenza viruses (Tamiflu hard capsules and powder for oral suspension). It is used to treat and prevent influenza.

What it is used for

  • Treatment of influenza in adults and children including full-term neonates with typical symptoms when influenza is circulating; efficacy shown when started within two days
  • Post-exposure prevention in individuals 1 year or older after contact with an influenza case
  • In exceptional situations seasonal prevention from 1 year; during a pandemic also post-exposure prevention under 1 year
  • Not a substitute for influenza vaccination

Dosing

  • Adults and adolescents 13 years and over, treatment: 75 mg twice daily for 5 days (10 days if immunocompromised), started within the first two days
  • Post-exposure prevention: 75 mg once daily for 10 days; during a community outbreak up to 6 weeks
  • Children 1 year or older: weight-based, e.g. 10 kg to 15 kg 30 mg, > 15 kg to 23 kg 45 mg, > 23 kg to 40 kg 60 mg, > 40 kg 75 mg (treatment twice daily)
  • Infants 0 to 12 months (treatment): 3 mg/kg twice daily
  • Renal impairment (treatment): creatinine clearance > 30 to 60 ml/min 30 mg twice daily, > 10 to 30 ml/min 30 mg once daily; ≤ 10 ml/min not recommended

Contraindications

  • Hypersensitivity to the active substance or any excipient

Important warnings

  • Effective only against illness caused by influenza viruses; not a substitute for vaccination
  • Susceptibility of circulating strains is highly variable
  • Efficacy in immunocompromised patients not firmly established
  • Neuropsychiatric events, especially in children and adolescents; monitor closely for behavioural changes

Side effects

  • Adults/adolescents: most common nausea and vomiting, mostly once on day 1 or 2
  • Children: most common vomiting
  • Rare serious: anaphylactic reactions, hepatic disorders, angioneurotic oedema, Stevens-Johnson syndrome, toxic epidermal necrolysis, GI bleeding, neuropsychiatric disorders

Interactions

  • Clinically significant interactions via CYP450 are unlikely
  • Probenecid causes an approximate 2-fold increase in active metabolite exposure; no adjustment with normal renal function

Pregnancy and breastfeeding

  • Pregnancy: large amount of data showing no malformative or feto/neonatal toxicity; may be considered if necessary after weighing benefit and risk
  • Breastfeeding: low levels in breast milk; administration may be considered
