Olanzapine is an antipsychotic. It is used for schizophrenia, manic episodes and prevention of recurrence in bipolar disorder; as a depot injection for maintenance treatment of schizophrenia.

What it is used for

  • Zyprexa tablets: schizophrenia, moderate to severe manic episode, and prevention of recurrence in bipolar disorder (adults)
  • Zyprexa powder for injection: rapid control of agitation in schizophrenia or manic episode when oral therapy is not appropriate
  • Zypadhera depot injection: maintenance treatment of schizophrenia in adults sufficiently stabilised on oral olanzapine

Dosing

  • Zyprexa oral, schizophrenia and recurrence prevention: starting dose 10 mg/day; mania: 15 mg/day as monotherapy or 10 mg/day in combination; range 5-20 mg/day
  • Zyprexa oral: consider a lower starting dose (5 mg) for those 65 and over and in renal or hepatic impairment
  • Zyprexa intramuscular: initial dose 10 mg, short-term use for up to three consecutive days; maximum daily dose across all formulations 20 mg

Contraindications

  • Hypersensitivity to the active substance or any excipient
  • Known risk of narrow-angle glaucoma

Important warnings

  • Not recommended in dementia-related psychosis: increased mortality and risk of cerebrovascular accident in elderly patients

  • Smoking and carbamazepine may induce metabolism via CYP1A2
  • Fluvoxamine (CYP1A2 inhibitor) markedly increases olanzapine levels; consider a lower starting dose
  • Caution with alcohol and medicines causing hypotension or sedation; injection with parenteral benzodiazepines not recommended
