Key facts
Naloxegol (Moventig) is a peripherally acting mu-opioid receptor antagonist used to treat opioid-induced constipation in adults.
What it is used for
- Opioid-induced constipation in adults who have had an inadequate response to laxatives.
Dosing
- Recommended dose: 25 mg once daily, in the morning on an empty stomach at least 30 minutes before or 2 hours after the first meal; with or without laxatives.
- Moderate or severe renal insufficiency: start 12.5 mg, increase to 25 mg if well tolerated.
- With moderate CYP3A4 inhibitors (e.g. diltiazem, verapamil): start 12.5 mg once daily, may increase to 25 mg; no adjustment with weak inhibitors.
- No adjustment in the elderly or mild to moderate hepatic impairment; not recommended in severe hepatic impairment.
- Stop when systemic opioid therapy is stopped. The tablet may be crushed and taken in water.
Contraindications
- Hypersensitivity to the active substance, any excipient or any other opioid antagonist.
- Known or suspected gastrointestinal obstruction or increased risk of recurrent obstruction.
- Cancer patients at heightened risk of GI perforation (GI tract or peritoneal malignancies, recurrent or advanced ovarian cancer, VEGF inhibitor treatment).
- Concomitant strong CYP3A4 inhibitors (e.g. clarithromycin, ketoconazole, itraconazole, ritonavir; large quantities of grapefruit juice).
Important warnings
- GI perforation, including fatal cases, in patients at increased risk; caution in severe peptic ulcer, Crohn's disease, diverticulitis or peritoneal metastases. Stop if unusually severe or persistent abdominal pain develops.
- With a disrupted blood-brain barrier naloxegol may enter the CNS (risk of withdrawal or reduced analgesia).
- Opioid withdrawal syndrome may occur.
Side effects
- Most common (≥ 5%): abdominal pain, diarrhoea, nausea, headache and flatulence; mostly mild to moderate, early in treatment and resolving with continued use.
- Others: vomiting, nasopharyngitis, hyperhidrosis, opioid withdrawal syndrome; not known: gastrointestinal perforation.
Interactions
- Ketoconazole increased naloxegol AUC 12.9-fold – strong CYP3A4 inhibitors are contraindicated.
- Moderate CYP3A4 inhibitors: lower starting dose (see dosing); for CYP3A4 inducers see the full text.
Pregnancy and breastfeeding
- Not recommended in pregnancy because of the potential for opioid withdrawal in the foetus.
