WARNING: TORSADES DE POINTES AND SUDDEN DEATH

  • Cases of Torsades de Pointes, cardiac arrest, and death have been reported with the use of a higher than recommended dosage of loperamide hydrochloride capsules (see WARNINGSand OVERDOSAGE).
  • Loperamide hydrochloride capsules are contraindicated in pediatric patients less than 2 years of age (see CONTRAINDICATIONS).
  • Avoid loperamide hydrochloride capsules dosages higher than recommended in adults and pediatric patients 2 years of age and older due to the risk of serious cardiac adverse reactions (see DOSAGE AND ADMINISTRATION).

What it is used for

  • Acute nonspecific diarrhea in patients 2 years of age and older
  • Chronic diarrhea in adults associated with inflammatory bowel disease
  • Reducing the volume of discharge from ileostomies

Dosing

  • Acute diarrhea, adults and children 13 years and older: initially 4 mg, then 2 mg after each unformed stool; maximum 16 mg daily
  • Children 2 to 12 years: first-day dosing by age and weight (e.g. eight to twelve years: 2 mg three times daily); liquid formulation for ages 2 to 5
  • Chronic diarrhea, adults: initially 4 mg, then 2 mg after each unformed stool; usual maintenance 4 mg to 8 mg daily, maximum 16 mg
  • No improvement after at least 10 days at 16 mg per day: further use unlikely to help
  • Avoid higher than recommended doses; no dose adjustment in the elderly

Contraindications

  • Children less than 2 years of age
  • Hypersensitivity to loperamide or any excipient
  • Abdominal pain in the absence of diarrhea
  • Acute dysentery with blood in stools and high fever; acute ulcerative colitis
  • Bacterial enterocolitis caused by invasive organisms (Salmonella, Shigella, Campylobacter); pseudomembranous colitis

Important warnings

  • Boxed warning: torsades de pointes, cardiac arrest and death with higher than recommended doses
  • Boxed warning: contraindicated in children under 2 years (respiratory depression, cardiac reactions)
  • Avoid with QT-prolonging drugs and in patients with QT risk factors

Interactions

  • CYP3A4 inhibitors (e.g. itraconazole), CYP2C8 inhibitors (e.g. gemfibrozil) and P-glycoprotein inhibitors (e.g. quinidine, ritonavir) increase loperamide exposure and cardiac risk
  • Avoid QT-prolonging drugs (e.g. Class IA or III antiarrhythmics, certain antipsychotics, moxifloxacin, methadone)
