Ivermectin (Stromectol) is an anthelmintic indicated, per the US prescribing information, for intestinal strongyloidiasis and onchocerciasis.

What it is used for

  • Intestinal (nondisseminated) strongyloidiasis due to Strongyloides stercoralis
  • Onchocerciasis due to Onchocerca volvulus (no activity against adult parasites)

Dosing

  • Strongyloidiasis: single oral dose of approximately 200 mcg/kg body weight; tablet number per weight table
  • Strongyloidiasis: additional doses generally not necessary; follow-up stool examinations to verify eradication
  • Onchocerciasis: single oral dose of approximately 150 mcg/kg; retreatment of individual patients may be considered at intervals as short as 3 months
  • Take on an empty stomach with water
  • Safety in pediatric patients under 15 kg not established

Contraindications

  • Hypersensitivity to any component of the product

Important warnings

  • Onchocerciasis: Mazzotti reaction (cutaneous and systemic) and ophthalmological reactions from dying microfilariae
  • Neurotoxicity including altered consciousness up to coma, confusion and death has been reported
  • Patients with hyperreactive onchodermatitis (sowda) may be more likely to have severe reactions

Side effects

  • Strongyloidiasis: dizziness, pruritus, nausea, diarrhea, fatigue, abdominal pain, somnolence, tremor, rash
  • Laboratory: ALT/AST elevation, decreased leukocyte count
  • Onchocerciasis: Mazzotti-type reactions and postural hypotension

Interactions

  • Warfarin: elevated INR rarely reported

Pregnancy and breastfeeding

  • Should not be used during pregnancy since safety has not been established
  • Excreted in human milk in low concentrations; treat nursing mothers only if the benefit outweighs the risk
