Key facts

Ibrutinib (IMBRUVICA) is an oral Bruton's tyrosine kinase inhibitor. It is used in adults with B-cell malignancies: mantle cell lymphoma, chronic lymphocytic leukaemia and Waldenström's macroglobulinaemia.

What it is used for

  • Mantle cell lymphoma (MCL): previously untreated (with chemoimmunotherapy) and relapsed or refractory (single agent)
  • Chronic lymphocytic leukaemia (CLL), as single agent or in combination
  • Waldenström's macroglobulinaemia (WM), as single agent or in combination
  • Exact indication wording: see the full product information

Dosing

  • MCL: 560 mg once daily; in untreated MCL alternating with R-CHOP/R-DHAP, then daily for 24 months
  • CLL and WM: 420 mg once daily, until disease progression or intolerance
  • CLL with venetoclax: 3 cycles of ibrutinib alone, then 12 cycles with venetoclax
  • With moderate CYP3A4 inhibitors reduce to 280 mg; with strong inhibitors reduce to 140 mg or withhold for up to 7 days
  • Withhold for serious cardiac or other toxicities and resume per the full product information

Contraindications

  • Hypersensitivity to the active substance or any of the excipients
  • Preparations containing St. John's Wort

Important warnings

  • Most common grade 3/4: neutropenia, lymphocytosis, thrombocytopenia, hypertension, pneumonia

Interactions
