Key facts

Ibandronic acid is a bisphosphonate that inhibits bone resorption. It is used for postmenopausal osteoporosis and for breast cancer with bone metastases and tumour-induced hypercalcaemia.

What it is used for

  • Bondronat (infusion and tablets): prevention of skeletal events (pathological fractures, bone complications requiring radiotherapy or surgery) in adults with breast cancer and bone metastases.
  • Bondronat (infusion): treatment of tumour-induced hypercalcaemia with or without metastases.
  • Bonviva (tablets and injection): treatment of osteoporosis in postmenopausal women at increased risk of fracture; reduction in vertebral fracture risk demonstrated.

Dosing

  • Bondronat infusion, bone metastases: 6 mg every 3-4 weeks, infused over at least 15 minutes (the short infusion time only with normal renal function or mild impairment). For renal impairment see the full text.
  • Bondronat infusion, tumour-induced hypercalcaemia: after adequate rehydration, 4 mg as a single dose in most patients with severe hypercalcaemia, 2 mg in moderate hypercalcaemia.
  • Bondronat 50 mg film-coated tablets: one tablet daily; moderate renal impairment (CLcr ≥30 and <50 mL/min) every second day, severe (CLcr <30 mL/min) once weekly.
  • Bonviva 150 mg film-coated tablets: one tablet once a month after an overnight fast (at least 6 hours) and 1 hour before the first food or drink; not recommended with creatinine clearance below 30 ml/min.
  • Bonviva 3 mg solution for injection: 3 mg intravenously over 15 - 30 seconds every three months, with calcium and vitamin D; not recommended with creatinine clearance below 30 ml/min.
  • Tablets: swallow whole upright with a glass of plain water, do not chew or suck; do not lie down for 60 minutes afterwards.

Contraindications

  • Hypersensitivity to ibandronic acid or to any of the excipients; hypocalcaemia.
  • Tablets additionally: oesophageal abnormalities delaying emptying (e.g. stricture, achalasia); inability to stand or sit upright for at least 60 minutes.

Important warnings

  • Correct existing hypocalcaemia before starting; ensure adequate calcium and vitamin D intake.
  • Oral use: upper gastrointestinal irritation, oesophagitis and oesophageal ulcers; follow the dosing instructions strictly.
  • Osteonecrosis of the jaw and of the external auditory canal; atypical femoral fractures, mainly with long-term treatment – report thigh, hip or groin pain.
  • Intravenous use: anaphylactic reaction/shock, including fatal events; avoid intra-arterial or paravenous administration.
  • In osteoporosis, re-evaluate the need for continued treatment periodically, particularly after 5 or more years.

Side effects

  • Most frequent (osteoporosis): arthralgia and influenza-like symptoms, usually with the first dose, short-lived and mild or moderate.
  • Hypercalcaemia treatment: most frequently a rise in body temperature.
  • Serious: anaphylactic reaction/shock, atypical femoral fractures, osteonecrosis of the jaw, ocular inflammation, gastrointestinal irritation (tablets).

Interactions

  • Tablets: food, milk and products containing calcium or other multivalent cations (aluminium, magnesium, iron) reduce absorption; keep the fasting interval.
  • Metabolic interactions via cytochrome P450 are not considered likely.

Pregnancy and breastfeeding

  • Bonviva is only for postmenopausal women and must not be taken by women of childbearing potential; not to be used during pregnancy.
  • Bondronat: for pregnancy and breast-feeding see the full text.
