Key facts

Fremanezumab is a humanised monoclonal antibody that binds calcitonin gene-related peptide (CGRP). It is used for migraine prophylaxis in adults.

What it is used for

  • Prophylaxis of migraine in adults who have at least 4 migraine days per month.

Dosing

  • Ajovy (225 mg pre-filled syringe or pen): two dosing options – 225 mg once monthly or 675 mg every three months, given subcutaneously.
  • When switching regimens, give the first dose of the new regimen on the next scheduled dosing date of the prior regimen.
  • Treatment benefit should be assessed within 3 months after initiation; the need to continue should be evaluated regularly thereafter.
  • Missed dose: resume as soon as possible; do not give a double dose.
  • No dose adjustment for the elderly or in mild to moderate renal or hepatic impairment. Inject into abdomen, thigh or upper arm; self-injection possible after training.

Contraindications

  • Hypersensitivity to the active substance or to any of the excipients.

Important warnings

  • Anaphylactic reactions have been reported rarely, mostly within 24 hours, some delayed; do not continue treatment after a serious hypersensitivity reaction.
  • Patients with certain major cardiovascular diseases were excluded from clinical studies; no safety data are available in these patients.
  • The name and batch number of the biological product should be recorded.

Side effects

  • Very common: injection site reactions – pain (24%), induration (17%), erythema (16%); common injection site pruritus (2%).
  • Uncommon: hypersensitivity reactions such as rash, pruritus, urticaria and swelling. Rare: anaphylactic reaction.

Interactions

  • No pharmacokinetic interactions are expected; acute migraine treatments (analgesics, ergots, triptans) and other preventives did not affect fremanezumab pharmacokinetics.

Pregnancy and breastfeeding

  • As a precaution, it is preferable to avoid use during pregnancy.
  • Breast-feeding: a risk cannot be excluded in the first days after birth; afterwards consider only if clinically needed.
