Key facts
Fenofibrate is a lipid-regulating agent of the fibrate class. Per the US label it is used with diet for hypercholesterolemia, mixed dyslipidemia and severe hypertriglyceridemia.
What it is used for
- Primary hypercholesterolemia or mixed dyslipidemia in adults, as adjunct to diet
- Severe hypertriglyceridemia in adults, as adjunct to diet
- Not shown to reduce coronary heart disease morbidity and mortality in type 2 diabetes
Dosing
- Take with meals; continue a lipid-lowering diet
- Hypercholesterolemia/mixed dyslipidemia: initial dose 160 mg once daily
- Severe hypertriglyceridemia: 54 mg to 160 mg per day, adjusted by lipid levels at 4 to 8 week intervals; maximum 160 mg once daily
- Mild to moderate renal impairment: start 54 mg per day; avoid in severe renal impairment
- Elderly: dose selection based on renal function
- Withdraw if no adequate response after two months at the maximum dose
Contraindications
- Severe renal impairment, including dialysis
- Active liver disease, including primary biliary cirrhosis and unexplained persistent liver function abnormalities
- Preexisting gallbladder disease
- Nursing mothers
- Hypersensitivity to fenofibrate or fenofibric acid
Important warnings
- Effect on coronary heart disease morbidity and mortality not established
- Hepatotoxicity; monitor liver tests
- Myopathy and rhabdomyolysis, higher risk in elderly patients and with diabetes, renal insufficiency or hypothyroidism
- Cholelithiasis
- Pancreatitis, hypersensitivity reactions, venothromboembolic disease
Side effects
- Increases in liver function tests were the most frequent events leading to discontinuation
- Others: abdominal pain and other gastrointestinal complaints
- Serious: hepatotoxicity, pancreatitis, hypersensitivity reactions, venothromboembolic disease
Interactions
- Coumarin anticoagulants: potentiated effect with prolonged PT/INR; reduce anticoagulant dose and check INR frequently
- Immunosuppressants (cyclosporine, tacrolimus) and nephrotoxic agents: risk of worsening renal function
Pregnancy and breastfeeding
- Limited data; use in pregnancy only if the potential benefit justifies the potential risk to the fetus
- Contraindicated in nursing mothers
