Key facts

Famotidine is a histamine-2 (H₂) receptor antagonist that reduces gastric acid. Per the US label it is used for gastric and duodenal ulcers and reflux disease.

What it is used for

  • Adults and children 40 kg and greater: active duodenal ulcer and active gastric ulcer
  • Symptomatic nonerosive gastroesophageal reflux disease (GERD)
  • Erosive esophagitis due to GERD
  • Adults only: pathological hypersecretory conditions (e.g. Zollinger-Ellison syndrome); reduction of the risk of duodenal ulcer recurrence

Dosing

  • Duodenal ulcer: 40 mg once daily or 20 mg twice daily, up to 8 weeks
  • Gastric ulcer: 40 mg once daily, up to 8 weeks
  • Nonerosive GERD: 20 mg twice daily, up to 6 weeks; erosive esophagitis: 20 mg or 40 mg twice daily, up to 12 weeks
  • Hypersecretory conditions: start 20 mg every 6 hours, adjust individually
  • Reduction of DU recurrence: 20 mg once daily
  • Creatinine clearance less than 60 mL/min: reduced dosing per the table in the full text; tablets not for children under 40 kg

Contraindications

  • History of serious hypersensitivity reactions (e.g. anaphylaxis) to famotidine or other H₂ receptor antagonists

Important warnings

  • CNS adverse reactions (confusion, delirium, hallucinations, seizures) in elderly patients and in moderate to severe renal impairment
  • Symptomatic response does not preclude gastric malignancy

Side effects

  • In 1% or more: headache, dizziness, constipation
  • Less common: fever, fatigue, palpitations, elevated liver enzymes, nausea, thrombocytopenia, rash, seizure, hallucinations

Interactions

  • Drugs dependent on gastric pH: concomitant dasatinib, delavirdine, cefditoren and fosamprenavir not recommended
  • Tizanidine (CYP1A2 substrate): avoid concomitant use; otherwise monitor for hypotension, bradycardia and excessive drowsiness

Pregnancy and breastfeeding

  • Data in pregnant women are insufficient to establish a drug-associated risk; no adverse developmental effects in animal studies
