WARNINGS

Etoposide should be administered under the supervision of a qualified physician experienced in the use of cancer chemotherapeutic agents. Severe myelosuppression with resulting infection or bleeding may occur.

Key facts

Etoposide is a cytotoxic anticancer drug (a podophyllotoxin derivative), here as oral capsules. Per the US prescribing information, it is used in combination with other chemotherapy as first-line treatment of small cell lung cancer.

What it is used for

  • Small cell lung cancer, first-line, in combination with other approved chemotherapeutic agents

Dosing

  • Oral dose is two times the IV dose rounded to the nearest 50 mg, i.e. two times 35 mg/m²/day for 4 days to 50 mg/m²/day for 5 days
  • Dose should be modified for the myelosuppressive effects of other drugs in the combination or of prior radiotherapy or chemotherapy
  • Renal impairment: creatinine clearance 15 to 50 mL/min – 75% of the dose initially; for creatinine clearance < 15 mL/min, further dose reduction should be considered
  • Capsules are stored refrigerated at 2° to 8°C

Contraindications

  • Previous hypersensitivity to etoposide or any component of the formulation

Important warnings

  • Boxed warning: to be given under the supervision of a physician experienced in cancer chemotherapy; severe myelosuppression with infection or bleeding may occur
  • Blood counts (platelets, hemoglobin, white cells and differential) before therapy and before each cycle; withhold therapy if platelets fall below 50,000/mm³ or neutrophils below 500/mm³
  • Acute leukemia has been reported rarely
  • Can cause fetal harm
  • Anaphylactic-like reactions

Side effects

  • Leukopenia (less than 4,000 WBC/mm³) in 60 to 91% and thrombocytopenia (less than 100,000 platelets/mm³) in 22 to 41%
  • Nausea and vomiting (31 to 43%), anorexia, diarrhea, mucositis
  • Reversible alopecia in up to 66% of patients, sometimes progressing to total baldness
  • Hypotension, allergic reactions, peripheral neurotoxicity

Interactions

  • Other myelosuppressive drugs in the combination: dose modification needed

Pregnancy and breastfeeding

  • Can cause fetal harm; teratogenic in mice and rats
  • Breastfeeding or the drug should be discontinued, depending on the importance of the drug to the mother
