Key facts
Ergocalciferol (vitamin D₂) is used, per the US prescribing information, to treat hypoparathyroidism, vitamin D resistant rickets and familial hypophosphatemia.
What it is used for
- Hypoparathyroidism.
- Refractory rickets (vitamin D resistant rickets).
- Familial hypophosphatemia.
Dosing
- The range between therapeutic and toxic doses is narrow; dosage must be individualized under close medical supervision.
- Vitamin D resistant rickets: 12,000 to 500,000 IU daily.
- Hypoparathyroidism: 50,000 to 200,000 IU daily together with calcium lactate 4 g six times per day.
- Check blood calcium and phosphorus every 2 weeks or more often if needed; bone X-rays every month until the condition is corrected and stabilized.
- Vitamin D from fortified foods and supplements should be taken into account; readjust the dose as soon as there is clinical improvement.
Contraindications
- Hypercalcemia, malabsorption syndrome.
- Abnormal sensitivity to the toxic effects of vitamin D, hypervitaminosis D.
Important warnings
- In vitamin D resistant rickets the therapeutic range is especially narrow; great care is needed to prevent serious toxic effects.
- In hyperphosphatemia (e.g. renal osteodystrophy), maintaining normal serum phosphorus is essential to prevent metastatic calcification.
- Adequate dietary calcium is necessary for a clinical response.
- Contains tartrazine (FD&C Yellow No. 5), which may cause allergic-type reactions including bronchial asthma in susceptible people.
- Effects can persist for two or more months after stopping treatment.
Side effects
- Adverse effects are those of hypervitaminosis D with hypercalcemia: anorexia, nausea, weakness, weight loss, vague aches and stiffness, constipation, anemia, mild acidosis.
- Impaired renal function with polyuria, nocturia, polydipsia, hypercalciuria, nephrocalcinosis and vascular calcification, up to irreversible renal insufficiency which may result in death.
- Soft-tissue calcification (heart, blood vessels, renal tubules, lungs); in children, slowed linear growth.
Interactions
- Mineral oil interferes with absorption of fat-soluble vitamins, including vitamin D.
- Thiazide diuretics may cause hypercalcemia in hypoparathyroid patients.
Pregnancy and breastfeeding
- Vitamin D in excess of the recommended dietary allowance should be avoided in normal pregnancy; safety above 400 IU daily has not been established.
- Caution in nursing women: large maternal doses caused hypercalcemia in a child.
