Key facts

Doxazosin is a quinazoline alpha-1 blocker. Per the US prescribing information, it is used for benign prostatic hyperplasia and for hypertension.

What it is used for

  • Signs and symptoms of benign prostatic hyperplasia (BPH)
  • Hypertension, to lower blood pressure

Dosing

  • BPH: start 1 mg once daily in the morning or evening; titrate at 1 to 2 week intervals to 2 mg, then 4 mg and 8 mg once daily; maximum 8 mg once daily
  • Hypertension: start 1 mg once daily; daily dosage may be doubled up to 16 mg once daily as needed
  • Monitor blood pressure for at least 6 hours after the first dose and each dose increase; after a pause of several days, restart with the initial regimen
  • Severe hepatic impairment (Child-Pugh Class C): not recommended

Contraindications

  • Hypersensitivity to doxazosin, other quinazolines (e.g. prazosin, terazosin) or any component

Important warnings

  • Postural hypotension and syncope, especially after the first dose or a dose increase; additive blood pressure lowering with PDE-5 inhibitors
  • Intraoperative floppy iris syndrome during cataract surgery; the surgeon should be prepared
  • Rule out prostate cancer before starting treatment for BPH
  • Priapism may occur

Side effects

  • BPH: dizziness 15.6%, fatigue 8%, somnolence 3%, edema 2.7%, dyspnea 2.6%, hypotension 1.7%
  • Hypertension: dizziness 19%, fatigue/malaise 12%, somnolence 5%, rhinitis 3%, polyuria 2%
  • Postural effects and edema appeared to be dose-related

Interactions

  • Strong CYP3A inhibitors may increase doxazosin exposure; monitor blood pressure
  • PDE-5 inhibitors: additive blood pressure lowering and symptomatic hypotension

Pregnancy and breastfeeding

  • Limited data in pregnant women are not sufficient to inform a drug-associated risk; untreated hypertension carries maternal risks
  • A single case study reports doxazosin in human milk
