Key facts

Digoxin is a cardiac glycoside. Per the US prescribing information (Lanoxin), it is used for mild to moderate heart failure and for rate control in chronic atrial fibrillation.

What it is used for

  • Mild to moderate heart failure in adults, where possible with a diuretic and an ACE inhibitor
  • Heart failure in pediatric patients (increases myocardial contractility)
  • Control of ventricular response rate in adults with chronic atrial fibrillation

Dosing

  • Dose individualised by body weight, age, renal function and concomitant drugs; toxic levels are only slightly above therapeutic levels
  • Optional loading: half the total loading dose first, then ¼ every 6-8 hours twice
  • Total oral loading dose in adults and children over 10 years: 10-15 mcg/kg
  • Starting maintenance in adults and children over 10 years with normal renal function: 3.4-5.1 mcg/kg/day once daily
  • Maintenance by lean body weight and renal function (table in the full text); pediatric doses in the full text
  • Consider interrupting or reducing the dose before electrical cardioversion

Contraindications

  • Ventricular fibrillation
  • Known hypersensitivity to digoxin or other digitalis preparations

Important warnings

  • Ventricular fibrillation in Wolff-Parkinson-White syndrome with atrial fibrillation
  • Severe sinus bradycardia, sinoatrial block and AV block may occur
  • Digoxin toxicity; narrow therapeutic index

Side effects

  • Adverse reactions are dose-dependent and mostly occur above therapeutic doses
  • Cardiac arrhythmias
  • Digoxin toxicity: hospitalization for suspected toxicity in the DIG trial 2% (placebo 0.9%)

Interactions

  • Narrow therapeutic index: monitor levels and toxicity when starting, adjusting or stopping interacting drugs
  • Digoxin is a P-glycoprotein substrate; P-gp inducers and inhibitors alter its pharmacokinetics
  • Drugs increasing digoxin concentrations by more than 50% are listed in a table in the full text

Pregnancy and breastfeeding

  • Decades of experience in pregnancy have not identified a drug-associated risk of major birth defects or miscarriage
