Key facts
Dexrazoxane is a topoisomerase II inhibitor that limits tissue damage from leaked anthracyclines. It is used in adults to treat anthracycline extravasation.
What it is used for
- Treatment of anthracycline extravasation in adults.
Dosing
- Savene (infusion): once daily for 3 consecutive days – 1000 mg/m2 on the first and second day, 500 mg/m2 on the third day.
- Start the first infusion as soon as possible, within the first six hours after the accident; subsequent infusions at the same hour (+/- 3 hours) as on the first day.
- For a body surface area of more than 2 m2, a single dose should not exceed 2000 mg.
- Moderate to severe renal impairment (creatinine clearance <40 mL/min): reduce the dose by 50%. Not recommended in hepatic impairment or in the elderly.
- Intravenous infusion over 1-2 hours into a large vein of a different extremity; remove cooling at least 15 minutes before.
Contraindications
- Hypersensitivity to the active substance or to any of the excipients.
- Women of childbearing potential not using contraception; breast-feeding.
- Concomitant vaccination with yellow fever vaccine.
Important warnings
- Regular local examination until resolution; not effective against extravasation of other vesicants (e.g. vincristine, mitomycin, vinorelbine).
- Its cytotoxicity adds to that of chemotherapy (reversible haematological toxicity with a nadir on days 11-12); monitor blood counts regularly.
- Liver dysfunction may occur; with known liver disorders, test liver function before each administration. Monitor patients with impaired renal function for haematological toxicity.
- Anaphylactic reactions, including angioedema, bronchospasm, hypotension and loss of consciousness.
- The solvent contains potassium and sodium; monitor potassium closely in patients at risk of hyperkalaemia.
Side effects
- Most common: nausea/vomiting, bone marrow suppression (neutropenia, thrombocytopenia), injection site reactions, diarrhoea, stomatitis and increased transaminases.
- In the studies: nausea/vomiting in about one third, neutropenia and thrombocytopenia in about half of patients; all adverse reactions were rapidly reversible.
- Very common: postoperative infection.
Interactions
- Yellow fever vaccine contraindicated; other live attenuated vaccines not recommended.
- DMSO should not be used; combination with phenytoin not recommended.
- Ciclosporin, tacrolimus: excessive immunosuppression with risk of lymphoproliferative disease.
Pregnancy and breastfeeding
- Men and women must use effective contraception during and up to 6 months after treatment (mutagenic potential). Breast-feeding is contraindicated.
