Key facts

Cefixime is, per the US prescribing information, an oral cephalosporin antibiotic used to treat bacterial infections such as urinary tract infections, otitis media, pharyngitis and uncomplicated gonorrhea.

What it is used for

  • Uncomplicated urinary tract infections caused by Escherichia coli and Proteus mirabilis.
  • Otitis media, pharyngitis and tonsillitis, and acute exacerbations of chronic bronchitis caused by susceptible organisms.
  • Uncomplicated gonorrhea (cervical/urethral).
  • Each in adults and children six months of age or older.

Dosing

  • Adults: 400 mg daily, e.g. as one 400 mg capsule; uncomplicated cervical/urethral gonorrhea: a single 400 mg dose; may be taken without regard to food.
  • Children 6 months or older: 8 mg/kg/day of suspension, once daily or as 4 mg/kg every 12 hours; weight-based dose table in the full text; children over 45 kg or older than 12 years receive the adult dose.
  • Otitis media should be treated with the suspension; tablets or capsules should not be substituted.
  • Streptococcus pyogenes infections: treat for at least 10 days.
  • Renal impairment: normal dose at creatinine clearance of 60 mL/min or greater; lower doses below that and on hemodialysis or peritoneal dialysis – see the full text.

Contraindications

  • Known allergy to cefixime or other cephalosporins.

Important warnings

  • Hypersensitivity reactions: anaphylactic/anaphylactoid reactions including shock and fatalities; caution in penicillin-sensitive patients.
  • Clostridioides difficile-associated diarrhea, from mild diarrhea to fatal colitis, possibly also after therapy ends.
  • Dose adjustment in renal impairment and dialysis; monitor dialysis patients carefully.
  • Prothrombin activity may fall, especially in renal or hepatic impairment, poor nutrition or anticoagulant therapy.
  • Use without a proven or strongly suspected bacterial infection increases the risk of drug-resistant bacteria.

Side effects

  • Most common were gastrointestinal events (30% of adults): diarrhea 16%, loose or frequent stools 6%, nausea 7%, flatulence 4%, abdominal pain 3%, dyspepsia 3%.
  • Less common (less than 2%): pseudomembranous colitis, rash, urticaria, drug fever, pruritus, angioedema, erythema multiforme, Stevens-Johnson syndrome.
  • Transient rises in liver enzymes, hepatitis, jaundice; transient rises in BUN or creatinine, acute renal failure.

Interactions

  • Carbamazepine: elevated carbamazepine levels reported; drug monitoring may help.
  • Warfarin and anticoagulants: increased prothrombin time, with or without bleeding.
  • False-positive urine ketone tests with nitroprusside-based tests.

Pregnancy and breastfeeding

  • Observational data on cephalosporins including cefixime have not established a drug-associated risk of major birth defects, miscarriage or adverse outcomes.
  • No data on presence in human milk; cefixime is present in animal milk.
