Key facts
Caspofungin (Cancidas) is an echinocandin antifungal given by intravenous infusion. It is used for invasive candidiasis, invasive aspergillosis and empirical therapy in febrile neutropenia.
What it is used for
- Invasive candidiasis in adults and paediatric patients.
- Invasive aspergillosis in adults and paediatric patients refractory to or intolerant of amphotericin B, lipid formulations of amphotericin B and/or itraconazole.
- Empirical therapy for presumed fungal infections (such as Candida or Aspergillus) in febrile, neutropaenic adults or paediatric patients.
Dosing
- Adults: single 70 mg loading dose on Day 1, then 50 mg daily; in patients weighing more than 80 kg, 70 mg daily after the loading dose.
- Children (12 months to 17 years): based on body surface area, 70 mg/m2 on Day 1 (not more than 70 mg), then 50 mg/m2 daily (not more than 70 mg daily).
- Moderate hepatic impairment (Child-Pugh 7 to 9): 35 mg daily after a 70 mg loading dose; no experience in severe hepatic impairment.
- With enzyme inducers (e.g. rifampicin, efavirenz, nevirapine, phenytoin, carbamazepine, dexamethasone), consider increasing to 70 mg daily in adults.
- Given as a slow infusion over approximately 1 hour once daily; treatment duration in the full text.
Contraindications
- Hypersensitivity to the active substance or any of the excipients.
Important warnings
- Anaphylaxis has been reported; caspofungin should then be discontinued. Possibly histamine-mediated reactions (rash, facial swelling, angioedema, pruritus, bronchospasm) can occur.
- Less common non-Candida yeasts and non-Aspergillus moulds may not be covered.
- Close monitoring of liver enzymes should be considered with concomitant cyclosporin.
- Toxic epidermal necrolysis and Stevens-Johnson syndrome have been reported.
Side effects
- Common: headache, phlebitis, dyspnoea, nausea, diarrhoea, vomiting, elevated liver values, rash, pruritus, hypokalaemia, decreased haemoglobin and white blood cell count.
- In invasive aspergillosis, pulmonary oedema, ARDS and radiographic infiltrates were also reported.
Interactions
- Cyclosporin increases caspofungin AUC by approximately 35 %; transient transaminase increases may occur.
- Caspofungin reduced tacrolimus trough concentrations by 26 %; monitor tacrolimus levels and adjust the dose.
- Inducers of metabolic enzymes may decrease caspofungin exposure.
Pregnancy and breastfeeding
- Should not be used during pregnancy unless clearly necessary.
- Women receiving caspofungin should not breast-feed.
