Key facts

Carvedilol is a beta-blocker with additional alpha-1-blocking activity used for chronic heart failure, left ventricular dysfunction after myocardial infarction and hypertension.

What it is used for

  • Mild-to-severe chronic heart failure of ischemic or cardiomyopathic origin, usually added to diuretics, ACE inhibitors and digitalis
  • Left ventricular dysfunction (ejection fraction ≤ 40%) in clinically stable patients after myocardial infarction
  • Essential hypertension, alone or in combination

Dosing

  • Taken with food to reduce orthostatic effects; dosage individualized
  • Heart failure: start 3.125 mg twice daily for 2 weeks, then 6.25, 12.5 and 25 mg twice daily over intervals of at least 2 weeks
  • Heart failure: maximum 50 mg twice daily in patients over 85 kg with mild-to-moderate heart failure
  • After myocardial infarction: start 6.25 mg twice daily, increase after 3 to 10 days to 12.5 mg, target 25 mg twice daily
  • Hypertension: start 6.25 mg twice daily, increased every 7 to 14 days as needed; total daily dose not above 50 mg
  • Dose reduced if heart rate falls below 55 beats per minute

Contraindications

  • Bronchial asthma or related bronchospastic conditions
  • Second- or third-degree AV block, sick sinus syndrome, severe bradycardia (without a permanent pacemaker)
  • Cardiogenic shock or decompensated heart failure requiring intravenous inotropes
  • Severe hepatic impairment
  • History of serious hypersensitivity to carvedilol

Important warnings

  • No abrupt discontinuation in coronary artery disease: risk of worsening angina, myocardial infarction and ventricular arrhythmias; taper over 1 to 2 weeks when possible
  • Bradycardia; dose reduced if pulse falls below 55 beats per minute
  • Hypotension and dizziness, particularly at initiation and dose increases
  • Fluid retention or worsening heart failure may occur during up-titration

Side effects

  • Dizziness, lightheadedness and rarely syncope after initiation or dose increases
  • Bradycardia and hypotension
  • Fluid retention

Interactions

  • Potent CYP2D6 inhibitors (e.g. quinidine, fluoxetine, paroxetine, propafenone) may increase blood levels
  • Catecholamine-depleting drugs (e.g. reserpine, MAO inhibitors): observe for hypotension and severe bradycardia
  • Diuretics: additive effects and exaggerated orthostatic component

Pregnancy and breastfeeding

  • Data insufficient; beta-blockers in the third trimester may increase the risk of hypotension, bradycardia, hypoglycemia and respiratory depression in the neonate
