Key facts
Cabergoline is an ergot-derived dopamine agonist. Per the US prescribing information, it is used to treat hyperprolactinemic disorders, either idiopathic or due to pituitary adenomas.
What it is used for
- Hyperprolactinemic disorders, idiopathic or due to pituitary adenomas
- Not indicated for the inhibition or suppression of physiologic postpartum lactation
Dosing
- Initially 0.25 mg twice a week
- May be increased by 0.25 mg twice weekly up to 1 mg twice a week according to the serum prolactin level, no more rapidly than every 4 weeks
- Before starting, a cardiovascular evaluation should be performed and echocardiography considered; on long-term treatment, cardiac status should be assessed periodically
- After a normal prolactin level has been maintained for 6 months, cabergoline may be discontinued with periodic prolactin monitoring
- Hepatic impairment: caution and careful monitoring; elderly patients: usually start at the low end of the dosing range
Contraindications
- Uncontrolled hypertension
- Known hypersensitivity to ergot derivatives
- History of cardiac valvular disorders (valvulopathy of any valve)
- History of pulmonary, pericardial or retroperitoneal fibrotic disorders
Important warnings
- Cardiac valvulopathy: all patients should have a cardiovascular evaluation including echocardiogram; do not treat if valvular disease is found
- Other fibrotic complications (pulmonary, pericardial, retroperitoneal)
- Pregnancy-induced hypertension (e.g. preeclampsia, eclampsia, postpartum hypertension): dopamine agonists should generally not be used unless the benefit outweighs the risk
- Orthostatic hypotension; caution with other blood-pressure-lowering medications
- Impulse control/compulsive behaviors such as pathological gambling, increased libido and hypersexuality
Side effects
- Most common in a placebo-controlled study: nausea (27%), headache (26%), dizziness (15%), constipation (10%), asthenia (9%) and fatigue (7%)
- Also: abdominal pain, somnolence, postural hypotension, depression
- Serious: cardiac valvulopathy and fibrotic disorders
Interactions
- Should not be given with D2 antagonists such as phenothiazines, butyrophenones, thioxanthenes or metoclopramide
- Caution with other drugs that lower blood pressure
Pregnancy and breastfeeding
- No adequate and well-controlled studies in pregnant women; should be used during pregnancy only if clearly needed
- Interferes with lactation; should not be given to postpartum women who are breastfeeding or planning to breastfeed
