Key facts

Brinzolamide (Azopt) is a sulphonamide carbonic anhydrase inhibitor given as eye drops to lower elevated intraocular pressure.

What it is used for

  • Decrease of elevated intraocular pressure in ocular hypertension.
  • Open-angle glaucoma: as monotherapy in adults unresponsive to beta-blockers or in whom beta-blockers are contraindicated, or as adjunctive therapy to beta-blockers or prostaglandin analogues.

Dosing

  • One drop in the conjunctival sac of the affected eye(s) twice daily; some patients may respond better to one drop three times a day.
  • The dose should not exceed one drop in the affected eye(s) three times daily.
  • Shake the bottle well before use; nasolacrimal occlusion or gently closing the eyelid after instillation reduces systemic absorption.
  • If several eye medicines are used, administer them at least 5 minutes apart; eye ointments last.
  • No dose adjustment in the elderly; not recommended in hepatic impairment or in children and adolescents.

Contraindications

  • Hypersensitivity to the active substance or any excipient.
  • Known hypersensitivity to sulphonamides.
  • Severe renal impairment (creatinine clearance < 30 ml/min).
  • Hyperchloraemic acidosis.

Important warnings

  • Absorbed systemically; sulphonamide-type reactions including Stevens-Johnson syndrome and toxic epidermal necrolysis may occur - withdraw immediately if serious reactions appear.
  • Caution in patients at risk of renal impairment because of possible metabolic acidosis.
  • Concomitant use with oral carbonic anhydrase inhibitors is not recommended.
  • Contains benzalkonium chloride, which can cause keratopathy and discolour soft contact lenses; remove lenses before instillation and wait at least 15 minutes before reinsertion.
  • Careful monitoring in compromised corneas (e.g. diabetes mellitus, corneal dystrophies); not recommended in narrow-angle glaucoma.
  • May impair tasks requiring mental alertness or physical coordination.

Side effects

  • Most frequent: dysgeusia (6.0%, bitter or unusual taste) and temporary blurred vision (5.4%) upon instillation.
  • Common: eye irritation, eye pain, foreign body sensation, ocular hyperaemia.
  • Uncommon include corneal erosion, keratitis, headache, dizziness and bradycardia.

Interactions

  • CYP3A4 inhibitors (e.g. ketoconazole, itraconazole, clotrimazole, ritonavir) are expected to inhibit brinzolamide metabolism; caution advised.
  • Possible additive effects with oral carbonic anhydrase inhibitors; combination not recommended.

Pregnancy and breastfeeding

  • Not recommended during pregnancy or in women of childbearing potential not using contraception.
  • For breast-feeding, see the full text.
