Key facts

Bevacizumab is a monoclonal antibody against vascular endothelial growth factor (VEGF). As an infusion (Avastin) it is used in combination regimens for several metastatic cancers; as an intravitreal injection (Lytenava) for wet age-related macular degeneration.

What it is used for

  • Avastin: metastatic carcinoma of the colon or rectum with fluoropyrimidine-based chemotherapy (adults)
  • Avastin: first-line metastatic breast cancer with paclitaxel or capecitabine
  • Avastin: advanced non-small cell lung cancer other than predominantly squamous, with platinum-based chemotherapy, or with erlotinib for EGFR-activating mutations
  • Avastin: further indications (including gynaecological cancers) in the full text
  • Lytenava: neovascular (wet) age-related macular degeneration in adults

Dosing

  • Avastin, metastatic colorectal cancer: 5 mg/kg or 10 mg/kg every 2 weeks, or 7.5 mg/kg or 15 mg/kg every 3 weeks, as intravenous infusion
  • Avastin, metastatic breast cancer: 10 mg/kg every 2 weeks or 15 mg/kg every 3 weeks
  • Avastin, NSCLC with platinum-based chemotherapy: 7.5 mg/kg or 15 mg/kg every 3 weeks for up to 6 cycles, then as a single agent
  • Avastin is continued until disease progression or unacceptable toxicity; further regimens in the full text
  • Lytenava: 1.25 mg by intravitreal injection every 4 weeks, monthly at first until maximum visual acuity, then individualised intervals

Contraindications

  • Hypersensitivity to the active substance or excipients
  • Avastin: hypersensitivity to CHO cell products or other recombinant human or humanised antibodies; pregnancy
  • Lytenava: active or suspected ocular or periocular infection; active intraocular inflammation

Important warnings

  • Avastin: increased risk of gastrointestinal perforation and fistulae; discontinue permanently if GI perforation occurs
  • Avastin: haemorrhage, including pulmonary haemorrhage/haemoptysis, more common in NSCLC
  • Avastin: arterial thromboembolism; hypertension and proteinuria are likely dose-dependent
  • Lytenava: endophthalmitis, intraocular inflammation and retinal detachment after intravitreal injection; monitor intraocular pressure after injection
  • Lytenava: not to be given with other systemic or ocular anti-VEGF medicines

Side effects

  • Avastin: most frequent are hypertension, fatigue or asthenia, diarrhoea and abdominal pain
  • Avastin: most serious are GI perforations, haemorrhage and arterial thromboembolism
  • Lytenava: most frequent are conjunctival haemorrhage, vitreous floaters, eye pain and increased intraocular pressure
  • Lytenava: less frequent but more serious: transient blindness, endophthalmitis, intraocular inflammation

Interactions

  • Avastin: no clinically relevant effect of co-administered chemotherapy on bevacizumab pharmacokinetics
  • Lytenava: no interaction studies; not to be combined with other anti-VEGF medicines

Pregnancy and breastfeeding

  • Avastin: contraindicated in pregnancy; women of childbearing potential use effective contraception during and up to 6 months after treatment
  • Lytenava: not used in pregnancy unless benefit outweighs risk; contraception during treatment and for at least three months after the last dose
