Key facts
Atezolizumab is a monoclonal antibody against PD-L1 (immune checkpoint inhibitor). It is used in several cancers, including urothelial carcinoma and non-small cell lung cancer (NSCLC).
What it is used for
- Locally advanced or metastatic urothelial carcinoma in adults (monotherapy, after platinum-containing chemotherapy or if cisplatin-ineligible with PD-L1 ≥ 5%)
- Adjuvant treatment of early-stage NSCLC after complete resection and platinum-based chemotherapy, high risk of recurrence, PD-L1 ≥ 50% (not EGFR-mutant/ALK-positive)
- First-line metastatic non-squamous NSCLC in combination with bevacizumab, paclitaxel and carboplatin, or with nab-paclitaxel and carboplatin
- Further indications (including triple-negative breast cancer) are in the full text
Dosing
- Intravenous (Tecentriq 840 mg/1,200 mg concentrate): 840 mg every 2 weeks, or 1,200 mg every 3 weeks, or 1,680 mg every 4 weeks
- Subcutaneous (Tecentriq 1,875 mg solution for injection): 1,875 mg every 3 weeks; patients can switch between intravenous and subcutaneous use
- Duration by indication: until disease progression or unmanageable toxicity; adjuvant NSCLC for 1 year
- In combination therapy, atezolizumab is given first on the same day; partner-drug schedules are in the full text
- Patient selection by PD-L1 expression with a validated test where the indication requires it
Contraindications
- Hypersensitivity to atezolizumab or any excipient
Important warnings
- Immune-mediated adverse reactions, possibly affecting more than one body system and occurring after the last dose; withhold and give corticosteroids according to severity
- Permanently discontinue for recurrent Grade 3 and any Grade 4 immune-mediated reactions (except endocrinopathies controlled with replacement hormones)
- Taper corticosteroids over ≥ 1 month after improvement to Grade ≤ 1
- Traceability: record product name and batch number
Side effects
- Monotherapy, most common: fatigue (29.3%), decreased appetite, rash, nausea, cough, diarrhoea, pyrexia, dyspnoea, arthralgia
- In combination, most common: anaemia (36.8%), neutropenia, nausea, fatigue, alopecia, thrombocytopenia, peripheral neuropathy
- Subcutaneous form additionally: injection site reactions (4.5%)
- Immune-mediated adverse reactions (e.g. pneumonitis, hepatitis, colitis, endocrinopathies)
Interactions
- No metabolic drug interactions expected (cleared by catabolism)
- Avoid systemic corticosteroids or immunosuppressants before starting; they may be used afterwards to treat immune-mediated reactions
Pregnancy and breastfeeding
- No data in pregnant women; mechanism of action suggests potential foetal harm, including increased rates of abortion or stillbirth
- Women of childbearing potential must use effective contraception during and for 5 months after treatment
