Key facts

Amoxicillin is a penicillin-class (β-lactam) antibacterial. Per the US label it treats infections caused by susceptible β-lactamase–negative bacteria and is used in H. pylori eradication.

What it is used for

  • Infections of the ear, nose and throat (adults and pediatric patients)
  • Genitourinary tract infections
  • Skin and skin structure infections
  • Lower respiratory tract infections
  • Adults only: H. pylori infection and duodenal ulcer disease (triple or dual therapy with lansoprazole ± clarithromycin)

Dosing

  • Take at the start of a meal to minimize gastrointestinal intolerance
  • Adults and children over 40 kg, mild/moderate ENT, skin or genitourinary infection: 500 mg every 12 hours or 250 mg every 8 hours
  • Severe infections and lower respiratory tract: 875 mg every 12 hours or 500 mg every 8 hours
  • Children aged 3 months and older under 40 kg: 25 mg/kg/day (every 12 hours) or 20 mg/kg/day (every 8 hours); severe infections 45 or 40 mg/kg/day
  • Infants under 12 weeks: upper dose 30 mg/kg/day divided every 12 hours
  • Streptococcus pyogenes infections: at least 10 days of treatment
  • H. pylori triple therapy: 1 gram amoxicillin, 500 mg clarithromycin and 30 mg lansoprazole twice daily for 14 days

Contraindications

  • Serious hypersensitivity reaction (e.g. anaphylaxis or Stevens-Johnson syndrome) to amoxicillin or other β-lactam antibacterials (penicillins, cephalosporins)

Important warnings

  • Serious and occasionally fatal anaphylactic reactions; inquire about previous hypersensitivity before starting
  • Severe cutaneous adverse reactions (SJS, TEN, DRESS, AGEP); discontinue if lesions progress
  • Drug-induced enterocolitis syndrome (DIES)
  • Clostridioides difficile-associated diarrhea (CDAD)

Side effects

  • Most common (greater than 1%): diarrhea, rash, vomiting, nausea
  • Triple therapy: diarrhea (7%), headache (6%), taste perversion (5%)
  • Serious: anaphylaxis, severe skin reactions, CDAD

Interactions

  • Probenecid: increased and prolonged amoxicillin blood levels
  • Oral anticoagulants: prolonged prothrombin time (increased INR) reported; monitoring needed
  • Allopurinol: increased incidence of rashes

Pregnancy and breastfeeding

  • Pregnancy: no evidence of fetal harm in animal studies; no adequate studies in pregnant women, use only if clearly needed
