Key facts

Allopurinol is a xanthine oxidase inhibitor that lowers uric acid production. Per the US prescribing information, it is used for gout, cancer-therapy-related hyperuricemia and recurrent calcium oxalate stones.

What it is used for

  • Adults with primary or secondary gout (attacks, tophi, joint destruction, uric acid lithiasis, nephropathy)
  • Adults and children with leukemia, lymphoma or solid tumors receiving cancer therapy that raises uric acid
  • Adults with recurrent calcium oxalate calculi and high uric acid excretion despite lifestyle changes
  • Not recommended for asymptomatic hyperuricemia

Dosing

  • Gout: start at 100 mg daily, increase weekly by 100 mg until serum uric acid is 6 mg/dL or less
  • Renal impairment: start at 50 mg daily with smaller increases (table in the full text)
  • Minimal effective dosage 100 mg to 200 mg daily, maximum 800 mg daily; doses above 300 mg given in divided doses
  • Mild gout usually 200 mg to 300 mg daily; moderate to severe tophaceous gout 400 mg to 600 mg daily
  • Flare prophylaxis with colchicine or an anti-inflammatory agent is recommended at initiation
  • Other regimens (cancer therapy, calculi) are in the full text

Contraindications

  • History of hypersensitivity reaction to allopurinol or any ingredient

Important warnings

  • Serious, sometimes fatal skin reactions (TEN, SJS, DRESS); discontinue permanently at first skin rash
  • HLA-B*58:01 carriers are at higher risk of hypersensitivity; use not recommended unless benefits clearly outweigh risks
  • Nephrotoxicity: monitor kidney function early in treatment
  • Hepatotoxicity and myelosuppression may occur
  • Possible effect on driving and use of machinery

Side effects

  • Most frequent adverse reaction: skin rash
  • Common (≥ 1%): diarrhea, nausea, increased alkaline phosphatase and AST/ALT, acute gout attacks
  • Less common (< 1%): including fever, headache, vasculitis, pericarditis, bradycardia

Interactions

  • Bendamustine, thiazide diuretics, ampicillin and amoxicillin may increase the risk of skin rash
  • With thiazides and impaired renal function, monitor kidney function and reduce the dose
  • Further clinically important interactions are in the full text

Pregnancy and breastfeeding

  • Based on animal findings, may cause fetal harm; allopurinol and oxypurinol cross the placenta
