Marsh-Oberhuber classification
Gastroenterology and hepatology
Also known as: modified Marsh classification · Marsh-Oberhuber-Klassifikation (Zöliakie) · Marsh-Oberhuber sınıflaması (çölyak) · Marsh-Oberhuber classification (coeliac disease histology)
What it is for
Histological grading of small-bowel (usually duodenal) biopsies in coeliac disease by intraepithelial lymphocytes (IEL), crypt hyperplasia and villous atrophy.
Classification
| Category | Description |
|---|---|
| Type 0 | normal mucosa |
| Type 1 | normal villous architecture with increased intraepithelial lymphocytes |
| Type 2 | increased IEL with crypt hyperplasia, villi normal |
| Type 3a | increased IEL, crypt hyperplasia and partial (mild) villous atrophy |
| Type 3b | as 3a with marked (subtotal) villous atrophy |
| Type 3c | as 3a with total villous atrophy |
| Type 4 | hypoplastic (atrophic) lesion: flat mucosa without increased IEL; rarely used today |
| Note | the simpler Corazza-Villanacci scheme groups these as grade A (non-atrophic) and B1/B2 (partial/total atrophy); IEL cut-offs differ between schemes. |
