FIGO 2023 endometrial cancer staging
Oncology, staging and radiology
Also known as: FIGO-Klassifikation Endometriumkarzinom (2023) · Endometriyum kanseri FIGO 2023 evrelemesi · FIGO 2023 staging of endometrial cancer
What it is for
Surgical-pathological staging of endometrial carcinoma by FIGO. New in 2023: histological type (non-aggressive = low-grade endometrioid, grade 1–2; aggressive = high-grade endometrioid, serous, clear cell, mixed, undifferentiated, carcinosarcoma and others), lymphovascular space involvement (LVSI) and, when known, molecular class enter stages I–II.
Classification
| Category | Description |
|---|---|
| I | confined to the corpus (and ovary in IA3). IA1 non-aggressive type limited to a polyp or the endometrium; IA2 non-aggressive, <50% myometrial invasion, no or focal LVSI; IA3 low-grade endometrioid carcinoma of uterus and ovary meeting defined criteria; IB non-aggressive, ≥50% myometrial invasion, no or focal LVSI; IC aggressive type limited to a polyp or the endometrium. |
| II | IIA cervical stromal invasion (non-aggressive); IIB substantial LVSI (non-aggressive); IIC aggressive type with any myometrial invasion. |
| III | IIIA1 ovary or fallopian tube (unless IA3); IIIA2 uterine subserosa or through the serosa; IIIB1 vagina and/or parametria; IIIB2 pelvic peritoneum; IIIC1 pelvic nodes and IIIC2 para-aortic nodes up to the renal vessels, each with i = micrometastasis, ii = macrometastasis. |
| IV | IVA bladder and/or bowel mucosa; IVB abdominal peritoneal metastasis beyond the pelvis; IVC distant metastasis (including nodes above the renal vessels, lung, liver, brain, bone). |
| Molecular class | POLE-mutated (POLEmut), mismatch-repair deficient (MMRd), no specific molecular profile (NSMP), p53-abnormal (p53abn). In stages I–II, POLEmut disease confined to the corpus or with cervical extension becomes IAm-POLEmut and p53abn disease with any myometrial invasion becomes IICm-p53abn; MMRd and NSMP are recorded (e.g. IIm-NSMP) without changing the stage; stages III–IV are not changed. |
| Previous edition | FIGO 2009 (still used in some countries and older reports): IA <50%, IB ≥50% myometrial invasion; II cervical stroma; IIIA serosa/adnexa; IIIB vagina/parametrium; IIIC1 pelvic, IIIC2 para-aortic nodes; IVA bladder/bowel mucosa; IVB distant metastasis including intra-abdominal disease. |
