FIGO 2018 cervical cancer staging

Oncology, staging and radiology

Also known as: FIGO-Klassifikation Zervixkarzinom (2018) · Serviks kanseri FIGO 2018 evrelemesi · FIGO 2018 staging of carcinoma of the cervix uteri

What it is for

Staging of cervical carcinoma by the International Federation of Gynecology and Obstetrics (FIGO). Since 2018 imaging and pathology may supplement clinical findings in all stages (pathology overrides imaging and clinical findings); lymphovascular space involvement does not change the stage, and the lateral extent of a lesion is no longer considered.

Classification

CategoryDescription
Iconfined to the cervix (extension to the corpus is disregarded).
IAmicroscopic invasive carcinoma, depth of invasion ≤5 mm: IA1 stromal invasion ≤3 mm; IA2 >3 to ≤5 mm.
IBinvasion >5 mm, limited to the cervix: IB1 ≤2 cm; IB2 >2 to ≤4 cm; IB3 >4 cm in greatest dimension.
IIbeyond the uterus but not the lower third of the vagina or the pelvic wall: IIA upper two thirds of the vagina, no parametrial involvement (IIA1 ≤4 cm, IIA2 >4 cm); IIB parametrial involvement not reaching the pelvic wall.
IIIIIIA lower third of the vagina; IIIB pelvic wall and/or hydronephrosis or non-functioning kidney; IIIC pelvic (IIIC1) and/or para-aortic (IIIC2) lymph node metastasis, irrespective of tumour size.
r / p notationIIIC is written with r when based on imaging and p when confirmed by pathology (e.g. IIIC1r, IIIC1p); isolated tumour cells do not change the stage; when in doubt the lower stage is assigned.
IVIVA spread to adjacent pelvic organs (biopsy-proven bladder or rectal mucosa; bullous oedema alone does not count); IVB distant organs.
