Das Wichtigste in Kürze

Tenofovirdisoproxil ist ein nukleotidischer Reverse-Transkriptase-Hemmer. Er wird zur Behandlung der HIV-1-Infektion (in Kombination) und der chronischen Hepatitis B bei Erwachsenen und Kindern eingesetzt.

Anwendung

  • HIV-1-Infektion bei Erwachsenen in Kombination mit anderen antiretroviralen Arzneimitteln
  • HIV-1-Infektion bei Kindern und Jugendlichen mit NRTI-Resistenz oder Toxizität gegenüber Erstlinienmitteln
  • Chronische Hepatitis B bei Erwachsenen mit kompensierter oder dekompensierter Lebererkrankung oder lamivudinresistentem HBV
  • Chronische Hepatitis B bei Kindern und Jugendlichen mit kompensierter Lebererkrankung und immunaktiver Erkrankung

Dosierung

  • Viread 245 mg (Jugendliche 12 bis < 18 Jahre ab 35 kg): 245 mg einmal täglich mit Nahrung.
  • Kinder 6 bis < 12 Jahre: 123 mg (17 bis < 22 kg), 163 mg (22 bis < 28 kg) bzw. 204 mg (28 bis < 35 kg) einmal täglich mit Nahrung.
  • Granulat 33 mg/g: Kinder 2 bis < 12 Jahre 6,5 mg/kg einmal täglich, sowie bei Schluckproblemen.
  • Erwachsene mit CrCl < 50 ml/min: reduzierte Tagesdosis mit Granulat empfohlen; bei 30–49 ml/min alternativ 245 mg alle 48 Stunden.
  • Behandlungsdauer bei Hepatitis B nach HBe-/HBs-Serokonversion (siehe Fachinformation).

Gegenanzeigen

  • Überempfindlichkeit gegen den Wirkstoff oder einen der sonstigen Bestandteile

Wichtige Warnhinweise

  • Nierenschäden (Niereninsuffizienz, Hypophosphatämie, proximale Tubulopathie/Fanconi-Syndrom); Nierenfunktion überwachen
  • Knochenveränderungen, selten mit Frakturen
  • Akute Hepatitis-Exazerbation während und nach Absetzen der Hepatitis-B-Therapie
  • Vor Beginn bei HBV-Infizierten HIV-Test anbieten; kein Schutz vor HBV-Übertragung
  • Hohe Versagerrate bei Dreifachkombination mit Lamivudin plus Abacavir oder Didanosin

Nebenwirkungen

  • HIV: meist leichte bis mittelschwere gastrointestinale Beschwerden
  • Hepatitis B: am häufigsten Übelkeit (5,4 %)
  • Selten Niereninsuffizienz; gelegentlich proximale Tubulopathie mit Knochenveränderungen

Wechselwirkungen

  • Nicht mit anderen tenofovirdisoproxil- oder tenofoviralafenamidhaltigen Arzneimitteln oder Adefovirdipivoxil kombinieren
  • Didanosin: gleichzeitige Gabe nicht empfohlen
  • Nephrotoxische Arzneimittel (z. B. Aminoglykoside, Amphotericin B, Foscarnet, Ganciclovir) vermeiden; Cidofovir kann Spiegel erhöhen
  • Geringes Potenzial für CYP450-Wechselwirkungen

Schwangerschaft und Stillzeit

  • Mehr als 1.000 Schwangerschaftsausgänge ohne Hinweis auf Fehlbildungen; Anwendung bei Bedarf möglich.
  • Gabe im dritten Trimenon kann zusammen mit Immunglobulin und Impfung des Kindes die HBV-Übertragung verringern.
