What it is for

The International Federation of Gynecology and Obstetrics (FIGO) 2015 consensus classifies the intrapartum cardiotocograph (CTG) into three tiers from the baseline fetal heart rate (FHR), its variability and decelerations.

Classification

CategoryDescription
Normalbaseline 110–160 bpm; variability 5–25 bpm; no repetitive decelerations (decelerations are repetitive when they occur with more than 50 % of contractions).
Suspiciouslacking at least one characteristic of normality, but with no pathological feature.
Pathologicalbaseline below 100 bpm; or reduced or increased variability, or a sinusoidal pattern; or repetitive late or prolonged decelerations for more than 30 minutes (more than 20 minutes if variability is reduced); or one prolonged deceleration lasting more than 5 minutes.
Terms usedtachycardia above 160 bpm, bradycardia below 110 bpm; reduced variability below 5 bpm for more than 50 minutes in baseline segments (or more than 3 minutes during decelerations); increased variability above 25 bpm for more than 30 minutes; prolonged deceleration lasting more than 3 minutes; sinusoidal pattern: smooth regular wave of 5–15 bpm amplitude at 3–5 cycles/min lasting more than 30 minutes, without accelerations.
Interpretation (FIGO)normal: no hypoxia/acidosis; suspicious: low probability of hypoxia/acidosis; pathological: high probability of hypoxia/acidosis.
Other systemsthe US NICHD three-category system (Category I–III, 2008) uses different criteria and is not interchangeable with FIGO.
