Surgical Apgar Score

Intraoperative 10-point score for 30-day major complications or death

Estimated blood loss
Lowest mean arterial pressure
Lowest heart rate
Fill in every field to see the result.

Uses the lowest values during the operation, so drug-induced bradycardia (β-blockers) earns points. Discrimination is moderate (c ≈ 0.7); it was proposed as an end-of-case flag for patients at higher risk, not as a stand-alone prognosis.

Interpretation

0–4 — high risk58.6 % had a major complication or died within 30 days in the general/vascular validation cohort (Gawande 2007).
5–6Intermediate risk; complication rates fall stepwise with each band.
7–8Around the median score; lower intermediate risk.
9–10 — low risk3.6 % major complication or death in the validation cohort.

What is the Surgical Apgar Score?

The Surgical Apgar Score rates three intraoperative measures — estimated blood loss, lowest mean arterial pressure and lowest heart rate — on a 0–10 scale. Gawande and colleagues derived it in 2007 in colectomy patients in Boston and validated it in general and vascular surgery. Low scores identify patients at high risk of major complications or death within 30 days.

When to use it

  • At the end of an operation, to flag patients at higher risk of major complications
  • Communicating intraoperative course at handover
  • Surgical quality monitoring

When not to use it

  • As the only prognostic tool — it ignores preoperative risk
  • Cardiac surgery and paediatric surgery, where it was not derived

Clinical pearls and limitations

  • Uses the lowest values during the operation, so drug-induced bradycardia (β-blockers) earns points. Discrimination is moderate (c ≈ 0.7); it was proposed as an end-of-case flag for patients at higher risk, not as a stand-alone prognosis.
  • A lower heart rate earns more points; pathological bradyarrhythmias score 0.
  • Preoperative β-blockade can raise the score without reflecting lower risk.
  • Validated internationally (including the WHO checklist cohort across eight countries), with moderate discrimination.

Frequently asked questions

What is a bad Surgical Apgar Score?

4 or less: in Gawande's validation cohort 58.6 % of such patients had a major complication or died within 30 days.

Which parameters make up the Surgical Apgar Score?

Estimated blood loss, lowest mean arterial pressure and lowest heart rate during surgery.

Why does a low heart rate give more points?

It was an empirical finding in the derivation data: lower intraoperative heart rates were associated with fewer complications. Tachycardia may reflect hypovolaemia or stress, but the score does not test the mechanism.
