Das Wichtigste in Kürze

Voriconazol ist ein Triazol-Antimykotikum mit breitem Wirkspektrum (Vfend). Es wird bei Erwachsenen und Kindern ab 2 Jahren zur Behandlung und Prophylaxe schwerer invasiver Pilzinfektionen eingesetzt.

Anwendung

  • Invasive Aspergillose
  • Candidämie bei nicht-neutropenischen Patienten; fluconazolresistente schwere invasive Candida-Infektionen (inkl. C. krusei)
  • Schwere Pilzinfektionen durch Scedosporium spp. und Fusarium spp.
  • Prophylaxe invasiver Pilzinfektionen bei allogenen Stammzelltransplantat-Empfängern mit hohem Risiko

Dosierung

  • Erwachsene, intravenös: Aufsättigung 6 mg/kg alle 12 Stunden (erste 24 Stunden), danach 4 mg/kg zweimal täglich
  • Erwachsene, oral ab 40 kg: Aufsättigung 400 mg alle 12 Stunden, danach 200 mg zweimal täglich (unter 40 kg: 200 mg bzw. 100 mg)
  • Bei unzureichendem Ansprechen oral bis 300 mg zweimal täglich (unter 40 kg 150 mg)
  • Kinder von 2 bis <12 Jahren: i.v. Aufsättigung 9 mg/kg, Erhaltung 8 mg/kg; oral 9 mg/kg zweimal täglich (maximal 350 mg)
  • Elektrolytstörungen vor und während der Therapie korrigieren; Behandlung über 180 Tage nur nach Nutzen-Risiko-Abwägung
  • Dosierung bei Prophylaxe, Leberfunktionsstörung und Wechselwirkungen siehe Volltext

Gegenanzeigen

  • Überempfindlichkeit gegen den Wirkstoff oder einen der sonstigen Bestandteile
  • CYP3A4-Substrate mit Risiko schwerer Reaktionen, z. B. Terfenadin, Astemizol, Cisaprid, Pimozid, Chinidin, Ivabradin, Mutterkornalkaloide, Sirolimus
  • Starke CYP3A4-Induktoren, die den Voriconazolspiegel deutlich senken (Liste im Volltext)

Wichtige Warnhinweise

  • QTc-Verlängerung und seltene Torsade-de-pointes; Vorsicht bei proarrhythmischen Zuständen
  • Lebertoxizität; Leberwerte überwachen
  • Vorsicht bei Überempfindlichkeit gegen andere Azole
  • Intravenöse Formulierung nicht länger als 6 Monate

Nebenwirkungen

  • Am häufigsten: Sehstörungen, Fieber, Hautausschlag, Erbrechen, Übelkeit, Durchfall, Kopfschmerzen
  • Periphere Ödeme, auffällige Leberwerte, Atemnot, Bauchschmerzen
  • Meist leicht bis mittelschwer

Wechselwirkungen

  • Wird über CYP2C19, CYP2C9 und CYP3A4 metabolisiert und hemmt diese; starker CYP3A4-Hemmer
  • Kann Spiegel von CYP3A4-Substraten deutlich erhöhen; Induktoren senken den Voriconazolspiegel
  • Mit Efavirenz: Voriconazol-Erhaltungsdosis erhöhen und Efavirenz-Dosis reduzieren (siehe Volltext)

Schwangerschaft und Stillzeit

  • In der Schwangerschaft nur, wenn der Nutzen das Risiko für den Fötus eindeutig überwiegt; wirksame Verhütung
  • Stillen bei Behandlungsbeginn beenden
