Wichtigster Warnhinweis (Boxed Warning)

- Fluorchinolone, einschließlich Levofloxacin, wurden mit behindernden und potenziell irreversiblen schwerwiegenden Nebenwirkungen in Verbindung gebracht, die gemeinsam aufgetreten sind [siehe Warnhinweise und Vorsichtsmaßnahmen (5.1)], darunter: Tendinitis und Sehnenruptur [siehe Warnhinweise und Vorsichtsmaßnahmen (5.2)] Periphere Neuropathie [siehe Warnhinweise und Vorsichtsmaßnahmen (5.3)] Wirkungen auf das Zentralnervensystem [siehe Warnhinweise und Vorsichtsmaßnahmen (5.4)]
Levofloxacin sofort absetzen und die Anwendung von Fluorchinolonen, einschließlich Levofloxacin, bei Patienten vermeiden, bei denen eine dieser schwerwiegenden Nebenwirkungen auftritt [siehe Warnhinweise und Vorsichtsmaßnahmen (5.1)]
- Fluorchinolone, einschließlich Levofloxacin, können bei Patienten mit Myasthenia gravis die Muskelschwäche verstärken. Levofloxacin bei Patienten mit bekannter Myasthenia gravis in der Anamnese vermeiden [siehe Warnhinweise und Vorsichtsmaßnahmen (5.5)].
- Da Fluorchinolone, einschließlich Levofloxacin, mit schwerwiegenden Nebenwirkungen in Verbindung gebracht wurden [siehe Warnhinweise und Vorsichtsmaßnahmen (5.1 bis 5.15)], ist Levofloxacin für folgende Indikationen Patienten vorzubehalten, für die keine alternativen Behandlungsmöglichkeiten bestehen: Unkomplizierte Harnwegsinfektion [siehe Anwendungsgebiete (1.12)] Akute bakterielle Exazerbation einer chronischen Bronchitis [siehe Anwendungsgebiete (1.13)] Akute bakterielle Sinusitis [siehe Anwendungsgebiete (1.14)]

- Fluoroquinolones, including levofloxacin, have been associated with disabling and potentially irreversible serious adverse reactions that have occurred together [see Warnings and Precautions (5.1)] , including: Tendinitis and tendon rupture [see Warnings and Precautions (5.2)] Peripheral neuropathy [see Warnings and Precautions (5.3)] Central nervous system effects[see Warnings and Precautions (5.4)]
Discontinue levofloxacin immediately and avoid the use of fluoroquinolones, including levofloxacin, in patients who experience any of these serious adverse reactions [see Warnings and Precautions (5.1)]
- Fluoroquinolones, including levofloxacin, may exacerbate muscle weakness in patients with myasthenia gravis. Avoid levofloxacin in patients with a known history of myasthenia gravis [see Warnings and Precautions (5.5)].
- Because fluoroquinolones, including levofloxacin, have been associated with serious adverse reactions [see Warnings and Precautions (5.1 to 5.15)] , reserve levofloxacin for use in patients who have no alternative treatment options for the following indications: Uncomplicated urinary tract infection [see Indications and Usage (1.12)] Acute bacterial exacerbation of chronic bronchitis [see Indications and Usage (1.13)] Acute bacterial sinusitis[see Indications and Usage (1.14)]

Levofloxacin ist ein Fluorchinolon-Antibiotikum. Laut US-Fachinformation wird es bei Erwachsenen zur Behandlung verschiedener bakterieller Infektionen eingesetzt, u. a. Pneumonie, Haut-, Harnwegs- und Prostatainfektionen sowie nach Milzbrand- oder Pestexposition.
Anwendung
- Nosokomiale und ambulant erworbene Pneumonie
- Komplizierte und unkomplizierte Haut- und Weichteilinfektionen; chronische bakterielle Prostatitis
- Komplizierte Harnwegsinfektionen und akute Pyelonephritis
- Inhalationsmilzbrand (nach Exposition) und Pest, auch bei Kindern
- Unkomplizierte Harnwegsinfektion, akute Exazerbation einer chronischen Bronchitis und akute Sinusitis nur ohne alternative Behandlungsoptionen
Dosierung
- Übliche Dosis 250 mg, 500 mg oder 750 mg einmal alle 24 Stunden, je nach Infektion
- Beispiele: nosokomiale Pneumonie 750 mg für 7 bis 14 Tage; unkomplizierte Harnwegsinfektion 250 mg für 3 Tage; chronische Prostatitis 500 mg für 28 Tage
- Kreatinin-Clearance unter 50 mL/min: Anpassung des Dosierungsschemas erforderlich (Tabelle im vollständigen Text)
- Kinder (Milzbrand, Pest): Dosierung nach Körpergewicht im vollständigen Text
- Antazida, Sucralfat, Eisen, Zink-Multivitamine mindestens zwei Stunden vor oder nach der Einnahme
Gegenanzeigen
- Bekannte Überempfindlichkeit gegen Levofloxacin oder andere Chinolone
Wichtige Warnhinweise
- Boxed Warning: behindernde, möglicherweise irreversible Nebenwirkungen (Tendinitis, Sehnenruptur, periphere Neuropathie, ZNS-Wirkungen); sofort absetzen
- Boxed Warning: kann Muskelschwäche bei Myasthenia gravis verstärken; bei bekannter Myasthenie meiden
- Überempfindlichkeitsreaktionen, Hepatotoxizität, Aortenaneurysma und -dissektion
- QT-Verlängerung, Blutzuckerstörungen, Photosensibilität, Clostridioides-difficile-assoziierte Diarrhö
- Bei Kindern vermehrt muskuloskelettale Ereignisse
Nebenwirkungen
- Am häufigsten: Übelkeit, Kopfschmerzen, Durchfall, Schlaflosigkeit, Verstopfung und Schwindel
- Schwerwiegend: Sehnenruptur, periphere Neuropathie, ZNS-Reaktionen, Leberschäden, Aortendissektion
Wechselwirkungen
- Magnesium- oder aluminiumhaltige Antazida, Sucralfat, Eisen, Zink, Didanosin: verminderte Aufnahme
- Warfarin: verstärkte Wirkung mit Blutungen berichtet; INR überwachen
- Antidiabetika: Hypo- und Hyperglykämie; NSAR: erhöhtes Risiko für ZNS-Stimulation und Krampfanfälle
Schwangerschaft und Stillzeit
- Veröffentlichte Daten zeigen kein arzneimittelbedingtes Risiko für schwere Fehlbildungen oder Fehlgeburten
- Geht in die Muttermilch über; Stillen während der Behandlung und zwei weitere Tage danach nicht empfohlen (alternativ Milch abpumpen und verwerfen)
