Das Wichtigste in Kürze
Allopurinol ist ein Xanthinoxidase-Hemmer, der die Harnsäurebildung senkt. Laut US-Fachinformation wird es bei Gicht, tumortherapiebedingter Hyperurikämie und rezidivierenden Calciumoxalatsteinen eingesetzt.
Anwendung
- Erwachsene mit primärer oder sekundärer Gicht (Anfälle, Tophi, Gelenkzerstörung, Harnsäuresteine, Nephropathie)
- Erwachsene und Kinder mit Leukämie, Lymphom oder soliden Tumoren unter Krebstherapie mit Anstieg der Harnsäure
- Erwachsene mit rezidivierenden Calciumoxalatsteinen und erhöhter Harnsäureausscheidung trotz Lebensstiländerung
- Nicht empfohlen bei asymptomatischer Hyperurikämie
Dosierung
- Gicht: Beginn mit 100 mg täglich, wöchentliche Steigerung um 100 mg bis Serumharnsäure 6 mg/dl oder weniger
- Bei Niereninsuffizienz: Beginn mit 50 mg täglich und kleinere Steigerungsschritte (Tabelle in der Fachinformation)
- Minimale wirksame Dosis 100 mg bis 200 mg täglich, Höchstdosis 800 mg täglich; Dosen über 300 mg aufgeteilt geben
- Leichte Gicht meist 200 mg bis 300 mg täglich, mittelschwere bis schwere tophöse Gicht 400 mg bis 600 mg täglich
- Zu Therapiebeginn Anfallsprophylaxe mit Colchicin oder einem Entzündungshemmer empfohlen

Gegenanzeigen
- Überempfindlichkeitsreaktion gegen Allopurinol oder einen Bestandteil in der Vorgeschichte
Wichtige Warnhinweise
- Schwere, teils tödliche Hautreaktionen (TEN, SJS, DRESS); bei erstem Hautausschlag dauerhaft absetzen
- Träger des HLA-B*58:01-Allels haben ein höheres Risiko für Überempfindlichkeit; Anwendung bei ihnen nicht empfohlen, außer der Nutzen überwiegt klar
- Nephrotoxizität: Nierenfunktion zu Therapiebeginn überwachen
- Hepatotoxizität und Myelosuppression möglich
- Mögliche Beeinträchtigung der Fahrtüchtigkeit
Nebenwirkungen
- Häufigste Nebenwirkung: Hautausschlag
- Häufig (≥ 1 %): Durchfall, Übelkeit, Anstieg von alkalischer Phosphatase und AST/ALT, akute Gichtanfälle
- Selten (< 1 %): u. a. Fieber, Kopfschmerz, Vaskulitis, Perikarditis, Bradykardie
Wechselwirkungen
- Bendamustin, Thiaziddiuretika, Ampicillin und Amoxicillin können das Risiko für Hautausschlag erhöhen
- Bei Thiaziden und eingeschränkter Nierenfunktion Nierenfunktion überwachen und Dosis reduzieren
